Plain ASCII text-file, one entry on each line (Windows style). Wild characters are allowed in the IP field. To ban someone you can either enter the man manually, or use the /ban command. To unban, use /unban or /unbanlast.

Structure

#IP, ban_duration - time in ticks(negative means permanent), ban_reason ("Banned by <admin>",Flood,etc)
